Sindrome hemofagocítico secundario: reporte de 5 casos.

dc.descriptionSecondary hemophagocytic syndrome (HFS) is an uncommon entity with a high mortality rate in adults, if no therapy is given. It is characterized by a severe hipercytokinemia due to a highly stimulated but ineffective immune system. The principal causes are infections, malignancy or autoimmune diseases. It appears as a serious illness, similar to a multiorgan failure. Treatment is not well defined. We report five patients with HFS, aged 17 to 51 years (three females). The etiology was onco-hematological in three patients. In two patients, the diagnosis was performed during necropsy. One case was due to cytomegalovirus (CMV) infection in a hepatic transplant patient and the other, due to parenteral lipid administration. All presented fever, cytopenia, hepatosplenomegaly and hemophagocytosis. Four of them required admission in an Intensive Care Unit. All received different treatment modalities. Only one survived. Median survival time was 75 days. In conclusion, HFS has different etiologies and a high mortality in adults.en-US
dc.descriptionIntroducción: El síndrome hemofagocítico (SHF) secundario  es una entidad poco frecuente cuyo desenlace es fatal sin tratamiento. Se produce por una severa hipercitoquinemia debido a un sistema  inmune altamente estimulado, pero inefectivo. Se desencadena principalmente por causa infecciosa, maligna o autoinmune. Se presenta generalmente como cuadros graves, similares a una falla multiorgánica. El tratamiento no está bien definido. Casos clínicos: Se presentan 5 pacientes, tres de sexo femenino, entre 17 y 51 años, y una mediana de 29 años. Las causas identificadas fueron hemato-oncológicas en 3 pacientes (2 diagnosticados post mortem), uno por infección por citomegalovirus (CMV) en una paciente con trasplante hepático y otro asociado a la administración de lípidos parenterales.  Todos presentaron  fiebre, citopenias, hepatoesplenomegalia y hemofagocitosis. Cuatro requirieron ingreso a unidad de paciente crítico. Los 5 pacientes recibieron diferentes esquemas de tratamiento. Sólo un paciente sobrevivió. El tiempo medio de sobrevida fue 75 días. Conclusión. Los casos presentados confirman que el SHF es una entidad  de etiología diversa y elevada mortalidad en adultos.es-ES
