| dc.description | We present a case of atrial flutter with a 2:1 block, associated with hydrops in one fetus of a triplet pregnancy, detected for a foetal tachycardia and confirmed by ecocardrography at 26 weeks of gestation. Digoxin was administered to the mother without success; flecainide was added on the 13th day of treatment, converting the arrythmia into sinus rhythm with resolution of the hydrops in utero. We discuss the utility of flecainide as a first line treatment and the need for postnatal antiarrythmic prophylactic, considering the favorable out come during this period. | en-US |
| dc.description | Se presenta un caso de flutter auricular, con bloqueo auriculoventricular 2:1, asociado a hídrops en uno de los fetos de un embarazo gemelar triple, pesquisado por taquicardia fetal y confirmado con ecocardiografía a las 26 semanas de gestación. Se administró digoxina a la madre, sin éxito, por lo que agregamos flecainida al décimo tercer día de tratamiento, logrando conversión a ritmo sinusal y regresión del hídrops dentro del útero. Se discute la utilidad del flecainide como primera línea para este tipo de pacientes y la necesidad de profilaxis antiarrítmica postnatal, considerando la favorable evolución en este período. | es-ES |
