dc.description | In April 2020, the pediatric multisystem inflammatory syndrome temporarily associated with COVID-19 (MIS-C) was described for the first time. MIS-C could have a severe course and may require critical care support.Objective: To describe the clinical, laboratory, and management characteristics of hospitalized children who meet MIS-C criteria with severe presentation in a pediatric critical patient unit.Patients and Method: Descriptive prospective study of children with severe MIS-C managed by treatment phases with immunoglobulin and methylprednisolone, according to their clinical response. Epidemiological, clinical, laboratory and imaging data were obtained. Phenotypes were classified into Kawasaki and not Kawasaki, comparing their findings.Results: 20 patients were analyzed, the median age was 6 years, 60% were female, and 40% presented comorbidity. SARS-CoV-2 was detected in 90% of the patients. They presented fever as the first symptom, followed by brief and early gastrointestinal symptoms (70%). 75% presented the Kawasaki phenotype. They evolved with lymphopenia, hypoalbuminemia, coagulation alterations, and elevated systemic and cardiac inflammatory parameters. 80% of the cases presented echocardiographic alterations and 90% shock that required critical care support. All the patients had a short and favorable evolution. All patients responded to the established therapy, but 40% required a second phase of treatment. There were no differences when comparing phenotypes. No deaths were reported.Conclusion: MIS-C is a new childhood disease whose presentation could be life-threatening. It requires early suspicion, immunomodulatory management, critical care support, and a multidisciplinary approach to obtain the best results and optimize its prognosis. | en-US |
dc.description | En abril de 2020 se describe por primera vez el síndrome inflamatorio multisistémico pediátrico temporalmente asociado al COVID-19 (SIM-C), el que puede evolucionar gravemente, con necesidad de soporte intensivo.Objetivo: Describir las características clínicas, de laboratorio y manejo de niños hospitalizados que cumplen con criterios de SIM-C de presentación grave en una unidad de paciente crítico pediátrico.Pacientes y Método: Estudio prospectivo descriptivo de niños con SIM-C grave manejados por fases de tratamiento con inmunoglobulina y metilprednisolona, según su respuesta clínica. Se obtuvieron datos epidemiológicos, clínicos, de laboratorio e imágenes. Se clasificaron fenotipos de presentación en Kawasaki y no Kawasaki, comparando sus hallazgos.Resultados: Se analizaron 20 pacientes, mediana de 6 años, 60% sexo femenino, 40% refirió comorbilidad. Se identificó SARS-CoV-2 en el 90%. Presentaron fiebre como primer síntoma, seguido por clínica gastrointestinal (70%) breve y precoz. 75% presentó fenotipo Kawasaki. Evolucionaron con linfopenia, hipoalbuminemia, alteraciones de coagulación y elevación de parámetros inflamatorios sistémicos y cardíacos. El 80% presentó alteraciones ecocardiográficas y el 90% presentó shock que requirió soporte intensivo, todos con evolución breve y favorable. Todos los pacientes respondieron a la terapia instaurada, pero un 40% requirieron una segunda fase de tratamiento. No hubo diferencias al comparar entre fenotipos. Ninguno falleció.Conclusión: El SIM-C es una nueva enfermedad infantil cuya presentación podría comprometer la vida de los pacientes. Requiere sospecha precoz, manejo inmunomodulador, soporte intensivo y un enfrentamiento multidisciplinario para obtener los mejores resultados y optimizar su pronóstico. | es-ES |
