El tratamiento restaurador de piezas dentarias  con endodoncia supone ciertas complicaciones, sobre todo cuando la restauración realizada no es la apropiada. Las indicaciones de tratamiento variarán de acuerdo al remanente dentario. En casos de que esta sea moderada, se opta por rehabilitar con un tipo de restauración indirecta, esta puede ser en resina, cerámica o metal. Una mala indicación de restauración llevará a ciertas complicaciones debido a que la rehabilitación de un diente tratado endodónticamente supone en la mayoría de los casos un reto importante para el odontólogo ya que debe solucionar la pérdida de estructura dentaria, la menor resistencia a la fractura ante las fuerzas oclusales y la necesidad de proporcionar retención suficiente al material restaurador que sustituirá la materia dental perdida. La fractura vertical corresponde a uno de los problemas recurrentes, los hallazgos clínicos que suponen el diagnostico de ella, generalmente no pueden ser comprobados  radiográficamente y se hace necesaria la exploración clínica.En ciertos casos se opta prematuramente por la exodoncia, las consecuencias sobre el equilibrio estomatognático de esta opción deben orientar al clínico a asegurar un adecuado diagnóstico y plan de tratamiento más conservador, aprovechando la amplia variedad de nuevos materiales y sistemas adhesivos, los que permiten la satisfacción del paciente y el clínico.Se presenta el caso de una paciente que presenta una fractura vertical en el diente 2.4, el que fue tratado conservadoramente, obteniendo resultados óptimos a nivel estético y funcional.Palabras clave: fractura vertical, tratamiento conservador, premolar, incrustación.Restorative treatment of endontic teeth supossed certain complications, especially when the restoration is not adequate. A bad indication of restoration will lead to certain complications due to the fact that the rehabilitation of a treated tooth endodónticamente supposes in the majority of the cases an important challenge for the odontologist since it must solve the loss of structure dentaria, the minor resistance to the fracture before the forces oclusales and the need to provide sufficient retention to the material restorer who will replace the dental lost matter.Dental cracks corresponds to one of the recurring problems, the clinical diagnosis of accounting for it, usually cannot be verified radiographically and clinical examination is necessary.In some cases it opt prematurely for the exodoncy, the implications of this option for the stomatognathic balance should guide the clinician to ensure proper diagnosis and more conservative treatment plan, using the wide variety of new materials and adhesive systems, which allow patient's and clinician's satisfaction.We report the case of a patient presenting a dental crack in the tooth 2.4, which was treated conservatively, obtaining optimal results in terms of aesthetics and functionality.Keywords: dental crack, conservative treatment, bicuspid, dental incrustation.
