dc.description | Background: Aging is the main risk factor to acquire breast cancer (BC). Nevertheless BC in elderly patients is sub-represented in clinical trials. Aim: To describe the clinical characteristics and long term treatment results of localized BC in older women. Material and Methods: Review of medical records of 65 women aged 70 to 88 years, with localized BC, treated with surgery, postoperative radiotherapy or systemic therapy at a Clinical Hospital in Chile. Results: The presence of symptoms or abnormal findings on physical examination were the main reasons for consulting in 65% of cases. Compared with tumors detected on physical examination, those detected using screening mammogram were smaller and were in stage 1 with a higher frequency (18 and 59% respectively, p < 0.01). The pathological type was luminal in 80% of cases. Overall survival was better for luminal pathological type. All patients completed radiotherapy without interruptions developing minor acute toxicity. The most common co-morbidity was high blood pressure occurring in 46% of patients. Thirteen percent of patients had three or more co-morbidities. After a median follow up of 7 years, 23 (35%) patients have died and the cause of death was BC in 43% of cases. Two patients died of lung cancer. No patient had a local breast relapse. Conclusions: Screening mammogram in older women detected smaller tumors and it was associated with a better survival. BC is the ultimate cause of death in approximately half of cases. | en-US |
dc.description | INTRODUCCION: El cáncer de mama (CM) es una de las principales causas de muerte por cáncer, siendo la edad su principal factor de riesgo. Sin embargo, pacientes adultos mayores (AM) están sub-representados en estudios clínicos. OBJETIVO: Describir las características clínicas, toxicidades y resultados a largo plazo de pacientes AM con CM localizado.MATERIAL Y METODOS: Revisamos características clínicas y evolución de pacientes mayores de 70 años con CM localizado, tratados con cirugía, radioterapia postoperatoria y/o terapia sistémica, atendidos en el Centro de Cáncer de la Pontificia Universidad Católica de Chile.RESULTADOS: Analizamos historias clínicas de 65 pacientes. Edad promedio: 76 años (70-88). La presencia de síntomas o hallazgos al examen físico fueron el motivo de consulta principal (65%). La hipertensión arterial fue la co-morbilidad más frecuente (46%). 13% tenían 3 o más co-morbilidades. Los tumores sólo detectados por mamografía de tamizaje fueron más pequeños que aquellos detectados por examen físico (59 % etapa I vs. 18 % etapa I respectivamente, p =0.0055). La mayoría de los tumores fueron definidos como luminales y recibieron tamoxifeno (80%). No hubo pacientes que suspendieran la radioterapia por toxicidad aguda. El subtipo luminal tiende a mostrar mejor sobrevida. Luego de 7 años de seguimiento, ninguna paciente ha desarrollado recaídas ipsilaterales; 23 (35%) pacientes han fallecido, 43% de ellos por CM. Dos pacientes fallecieron por cáncer pulmonar.CONCLUSIONES: El tamizaje de CM parece ser importante en AM. A pesar de la alta frecuencia de co-morbilidades el CM sigue siendo una importante causa de muerte en AM. | es-ES |
