dc.description | Background: In 2005, acute myocardial infarction (AMI) was included in a universal health plan (GES) to reduce inequity in care and optimize its diagnosis and treatment. Aim: To evaluate the effect of GES in risk factor control and therapeutic management among patients with AMI. Material and methods: A survey was conducted in 2008-2009 in six public hospitals. Patients were identified from a hospital based registry of AMI and evaluated one year later with laboratory tests and an interview. Results: The registry enrolled 534 patients with ST and non ST segment elevation myocardial infarction. Of these, 416 patients aged 63 ± 12 years (25% women) were evaluated one year later. Eighty three percent were evaluated by a cardiologist and 37% by a general practitioner. Twenty two percent were evaluated by a nurse and 22% by a nutritionist. At the moment of the interview, 9% smoked, 78% were overweight or obese, 24% performed moderate or vigorous physical activity ?150min/week, 60% had systolic pressure >130mm/Hg and 63% a diastolic pressure >80mm/Hg. In 30%, LDL cholesterol was >100mg/dl and in 43%, triglycerides were >150mm/dl. Twenty two percent were diabetic and among them, 52% had a glycosilated hemoglobin >7%. Forty five percent of non-diabetic patients had a fasting glucose >100mg/dl. Ninety three percent were in treatment with aspirin, 86% with statins, 66% with ?-blockers, and 73% with angiotensin converting enzyme inhibitors or angiotensin receptor blockers and 20% with clopidogrel. Conclusions: Despite the high proportion of patients in treatment with evidence-based therapy, many do not achieve the targets for risk factor control with the new health care model. | en-US |
dc.description | En 2005 el infarto agudo de miocardio (IAM) fue incluido en un régimen de garantías universales de salud (GES), cuyo objetivo fue mejorar la calidad de la atención de salud y disminuir inequidades. Objetivo: analizar el efecto de GES en el control de los factores de riesgo cardiovascular y tratamiento de pacientes post-infarto. Materiales y métodos: estudio realizado entre julio 2009 y marzo 2010 en seis hospitales públicos de Santiago y regiones en pacientes identificados del Registro hospitalario de infarto (GEMI) y controlados al año. Resultados: de 493 pacientes elegibles, en 416 (85,4%) se realizó seguimiento al año; la edad media fue 62,3± 12,3 años y 24,7% mujeres. En relación a la atención recibida en GES, 85% fue controlado al menos una vez por cardiólogo, 21,5% por enfermera y 22,0% por nutricionista. Al año: 9% fumaba, 78% presentaba IMC >25 kg/m2, 24% realizaba actividad física ?150 min/semana, 60% presentó presión arterial sistólica >130 mm/Hg y 63% diastólica >80 mm/Hg, en 30% el LDL fue >100 mg/dl y en 43% los triglicéridos >150 mm/dl. En diabéticos (22,4%) 52% presentó HbA1c >7% y 45% de los no diabéticos presentaron glicemias >100 mg/dl. El empleo de fármacos al año fue: aspirina 93%, estatinas 85,7%, betabloqueadores 65,6%, IECA o ARA II 72,9% y clopidogrel 33%. Conclusiones: en GES el tratamiento farmacológico se ajusta a las recomendaciones de guías clínicas, pero el logro de las metas establecidas para los factores de riesgo es insuficiente, al igual que la participación de un equipo de salud multidisciplinario. | es-ES |
