This article summarizes some of the most relevant approaches to tuberculosis (TB) presented at tje congress of the ALAT (Latin American Thoracic Association) 2025, which may be summarized in the following ; a) The need to identify the groups most vulnerable to the disease, all of whom should be treated if latent TB is demonstrated; b) the high mortality of Central Nervous System tuberculosis, which is why it is important to establish prolonged treatment with drugs that better penetrate the brain and meninges, along with treating the associated inflammation; c) the higher incidence of TB in pregnant women, especially in the postpartum period, TB under these conditions can be treated with standard regimens, except if there is resistance to these drugs. Some anti-TB drugs should also be considered harmful to the fetus, d) the potential capacity of TB to induce genomic changes that may culminate in the appearance of a neoplasm; e) the need for people living with HIV to identify and treat latent or active TB early, due to the high lethality of co-infection; f) TB risk in patients with diabetes mellitus with poor metabolic control who also have a slower cure and more adverse reactions to TB therapy.Este artículo resume algunos de los planteamientos más relevantes sobre tuberculosis (TB) pre­sentados en el Congreso ALAT 2025 y que pueden ser sintetizados en los siguientes: a) La necesidad de identificar a los grupos más vulnerables a la enfermedad todos los cuales deben ser tratados, si se demuestra una TB latente; b) La alta mortalidad de la TB del sistema nervioso central, por lo cual es importante instaurar un tratamiento prolongado con fármacos de mejor penetración al cerebro y meninges, junto con tratar la inflamación asociada; c) la mayor incidencia de TB en las embarazadas, especialmente en el puerperio. La TB en estas condiciones puede ser tratada con es­quemas habituales, excepto si existe resistencia a estos medicamentos. También se debe considerar que algunos medicamentos antituberculosos son nocivos para el feto; d) la potencial capacidad de la TB para inducir cambios genómicos que pueden culminar en la aparición de una neoplasia; e) la necesidad para las personas viviendo con VIH de identificar y tratar precozmente la TB latente o activa debido a la alta letalidad de la coinfección; f) el riesgo de TB en los enfermos con diabetes mellitus con deficiente control metabólico, quienes además, tienen una curación más lenta y más reacciones adversas a la terapia de la TB.
